FORMS FOR PAYING ASSESSMENTS ONLINE

 

Assesment fees are calculated at one half percent (0.005). For example, a gross sales figure of $100,000.00 multiplied by 0.005 would be $500.00. After calculating that figure, enter that number into the "Assesment Amount" field below and click the "Confirm Amount" button before proceeding. When you enter the number, do not include a comma. For example, you would enter $1,500.89 as "1500.89"

QCS Entities have the option to pay their semi-annual assessments through our website. Use the Gross Sales Assessment form, which is mailed to each entity 6 weeks before the Assessment due date, to calculate your Assessment amount. Note to entities who wish to pay through our website: you still must complete and return Gross Sales Assessment Form to QCS Office.
